Gear and Item Set Info
All sets listed give massive bonuses to Health Recovery and synergise well (Troll King and Orgnum). The jewelley and two armour pieces are easy to farm from dolmens and delves where as Orgnum is crafted and allows for easier freedom with traits.
Green Pact could be used instead of Bee Keeper to more health (around 42k) but personally I do not think it is worth it.
Heavy Armour only as need full benefit of constition passive.

This build works around stacking health recovery from all sources which I will list below. You will be able to achieve ariund 9k health recovery if below 60% health and 4.5k standard:

471 - 3 Jewellery glyphs
210 - The Steed Mundus Stone
129 - Orgnum set
758 - Beekeeper set
Major Fortitude (Green Dragon Blood) +20%
Nord, Khajiit, Orc passives +20%
Healthy Champion Points +25%
Now the magic happens, if you fall below 60% health and heal yourself you will will proc Troll King and Orgnum's Scales gaining:
1548 Health recovery
+50% Health recovery
Champion Point Tips
100 into Healthy is a must. the rest is down to preference. I would advise Hardy and Elemental Defender over Heavy Armour Foucs as this will provide more damage mitigation.
100 into Shadow Ward to reduce the cost of block
